Change order of pipeline and add author
Add explicit when rules

Remove session completely as that is handled by hydra
entirely. Remove tests, because login provider now only
servers as an interface that is used by hydra to authenticate
users. It will not have any other functionality (e.g. maintain
a session or single-sign out).
