feat: set logout endpoints for WordPress, Nextcloud and Grafana

Makes all the applications that support it use the logout endpoint.

What works:

Logging out of the following applications leads to a logout of Hydra and Kratos:

  • WordPress
  • Nextcloud
  • Grafana

There are issues with:

The documentation I added assumes that the Wekan and Zulip issue won't be fixed anytime soon, but also assumes that we'll fix dashboard#50 (closed) ASAP.
